2. Best Mailing address

3. Best Location address

4. Federal ID number (EIN) if you are an LLC or Corporation

5. Prior 12 months annual revenue (gross revenue)

6. Prior 12 months annual payroll

7. Estimated next 12 months annual revenue (gross revenue)

8. Estimated next 12 months annual payroll

9. Number of employees (including duties)

10. If you currently have insurance, we will need that information as well.
